Annual Report of Endowment Care - Due July 1, 2017
Effective with the 2016 Annual Report of Endowment Care, due in 2017, reports must be filed electronically (click this link to be redirected to the log-in ALDOI - Preneed Online). Upon clicking the link for ALDOI - Preneed Online, you must enter your Company number, which is your unique Cemetery Authority number. If your cemetery authority is licensed or registered to sell preneed, then you will use your Preneed Certificate of Authority number as your company number. Your log-in information was originally provided in an e-mail. If you did not receive this e-mail and do not have a company number and password or have forgotten it, contact the Preneed Division: (334)240-4420 or preneed@insurance.alabama.gov.
Due to changes in the form and the change from paper forms to online electronic forms, the reporting deadline has been extended. Endowment Care Reports are due by July 1, 2017. In 2017 ONLY, they will be accepted until August 31, 2017. Reports submitted by August 31, 2017, will be deemed to have been filed by April 1, 2017, as required by law. No Cemetery Authority applying to renew its Preneed Certificate of Authority, will be renewed until such time as its Annual Endowment Care Report and the related Annual Endowment Care Trust Statement has been received. The law requires all Endowment Care Cemeteries, whether licensed to sell preneed or not, to file a report and trust statement from the trustee on the trust’s activities for the reporting year, annually and within 90 days of the end of the reporting year (due April 1st). The report includes the annual trust statement provided by the trustee and if applicable a copy of each board member’s bond. At the Department’s discretion, the cemetery property sales log may be requested. Trust statements are required to be submitted electronically.
